Default inspection ticket

Found this assembly plant inspection ticket in the 69 GTO that I'm working on. Has anyone seen one of these before?

I haven't seen one exactly like that, but I found this two-sided piece in a '69 Grand Prix that was wired to the inner door structure. This was from the Pontiac Plant.

Inspection ticket is good; that one looks like it's from the body shop.

There would be one from the chassis line, motor line, trim line, final line and they would all be stapled together as the vehicle moved towards completion.
